A Few Things to Remember
1. Scheduling
Appointments are subject to availability. If you prefer specific times, such as evenings or Saturdays, I encourage you to schedule in advance.
2. Billing

Membership fee: $110/month

Visit fees are charged at time of service with your discount applied

Discounts don’t roll over month to month

Membership is for your visits only and can’t be combined with other discounts
3. Payment Method
If your card on file changes or expires, please update me so your membership stays active.
4. Cancellation
If you ever wish to cancel or take a break, just text or email me at least 5 days before your next billing date.
Membership fees are not refundable once charged.
5. Insurance
Memberships are designed for self-pay visits. Superbills can be provided upon request and will reflect your discounted rate. The membership fee itself is not reimbursable.

If you’d like to review your full membership agreement, you can log into your Jane account at any time — it’s always available in your signed forms.

🌿 Thrive Membership Details

$110/month — 20% off all visits during your billing month

Memberships renew automatically each month until canceled.

Cancel anytime by email or text at least 5 days before your renewal date to avoid future charges.

Your membership remains active through the end of your current billing cycle once cancellation is requested.

Membership fees are non-refundable once charged.

Discounts apply only to the member, for self-pay visits, and do not roll over to future months.

Appointments are subject to availability. Scheduling in advance is recommended to ensure your preferred appointment times.

Memberships are not transferable and cannot be combined with other discounts.

Eligibility: Memberships are available to existing patients only. Initial visits are not included in membership pricing.

Membership Term: Memberships are set for a 12-month term, but may be canceled at any time. Your membership will automatically renew for another 12-month period unless you choose to cancel at that time.

Insurance & Superbills: Memberships are designed for self-pay patients. I don’t bill insurance directly, but I can provide a superbill for you to submit to your insurance company. Superbills will reflect the discounted session rate, and your insurance may reimburse a percentage based on that amount. The membership fee itself is not reimbursable.

Stay enrolled for 3 consecutive months, and your discount increases to 22% off.

Stay for 6 consecutive months or longer, and enjoy 25% off every visit — as long as your membership remains active.

One-Year Appreciation Gift: Thrive members who stay enrolled for 1 full year receive a Heart Blossom Healing hoodie as a thank-you for their continued commitment to consistent care. Styles may vary with availability, but you’ll be able to choose your preferred color and size.
↘️ Downgrades & Cancellations

You may downgrade to the Nurture Membership or cancel your membership at any time. Your Thrive membership remains active through the end of your current billing cycle once the change or cancellation is requested.

If your membership is canceled, the discount simply returns to the starting 20% tier when you rejoin.
